Aging and Disability Resources

ILSNC has partnered with PASSAGES in designation as an Aging and Disability Resource Center (ADRC) through a grant administered through the California Department of Aging. The goal of the project is to enhance our capacity to expand consumer awareness of community based service and provide efficient and effective access to long term supportive services allowing older adults and persons with disabilities to make informed choices about these options. We are also involved in efforts to streamline the process by which consumers become enrolled in publicly financed supportive services and in assisting nursing home residents to prepare for and transition to more independent community living.

Our efforts include web-based access to information. We have developed a Resources page with a list of top rated web-based resources that will assist persons with disabilities and older adults in making informed decision about their long term supportive services options.
